Plan the route and handoff

Check live MTA service and the entrance you need, but call 911 when medical transport may be necessary. Bring identification, insurance information if available, a medication list, allergies, relevant records and a contact—but do not delay emergency help to assemble paperwork.

Before leaving any visit, ask what warning signs require escalation, where results will appear and who handles follow-up. Save discharge instructions and share them with the regular clinician when appropriate.
